1. Screw or nail head on surface of arch sheathing caught on chicken wire. 2. Form was too short and bags wrapped around edge of the face of the form. 3. Form lacked sufficient internal bracing to prevent deflection during construction.
Fortunately for you, you followed directions and screwed the face of the form exposed to the outside so in case of a problem you could dismantle it, right?
